Refresh Expired Tokens

Trigger token refresh for all at-risk and expired installations.

Called by the Supabase cron job every 15 minutes via HTTP POST. Protected by X-Cron-Secret header (cron path) or an admin session token. Fails closed — if CRON_SECRET is unset the cron path is disabled entirely and only admin sessions can trigger it.

Returns immediately and runs the actual refresh in the background so the Supabase cron HTTP call does not time out.
