Audit Call Completed Trigger Health

Trigger the call_completed trigger health sweeper.

Called by Supabase cron every 30 minutes. It quarantines locally-active trigger rows that GHL has already reported as inactive/deleted and marks installs degraded when no active call_completed triggers remain.
