List Usage Events

Paginated, filterable MCP call log for the Activity Log page.

Ordered ts DESC with uuid as a stable tiebreak. Cursor pagination via opaque base64(ts_iso|uuid). Org owners/admins see every row; plain members are scoped to their own user_uuid server-side — a member querying another user's rows gets an empty page rather than a 403 (parity with the chart endpoints).
