List question templates

Paginated list of the admin question-template library with each question's category name, newest edit first by default. sort takes column:ASC|DESC pairs over updatedAt, title, categoryName and type; filter is a JSON object matching title, type and categoryName case-insensitively by substring. count is at most 500 (15 by default). Rows carry the columns the list shows; the editor loads a question through GET /:id. Legacy answered every column in a meta envelope; this answers { data, metadata: { pagination } }.
