Synchronous audio/video transcription for a case file (SSE)

Transcribes an audio/video file in the context of a case file. Wraps /api/cases/[caseId]/transcribe. Firm-scoped OWNERSHIP (resolveCaseAccess). Input multipart/form-data. Returns text/event-stream (IngestProgress progress with done) — stream passthrough. Scope cases/mcp/*. The internal route debits 1 cr (standard) / 2 cr (premium) with refund; the v1 layer does not debit.
