Stream Founding

Re-attach after a refresh: tail the persisted transcript by seq.

Honest limitation (spec): live token deltas exist only on the original POST connection. A re-attached client catches up at persisted-message granularity until the next yield, which is fine because yields are where the founder is actually needed.
