Admin Onboarding Protocol Telemetry

Aggregates JSON-RPC protocol rejections (mcp_requests.response_type LIKE 'protocol_%'). These occur before an onboarding_sessions row exists — not joinable, but shows drop-off from malformed clients. Shown on the Admin → Onboarding tab.
