Start re-fetching a git-sourced app at its pinned ref and swapping it in.

Raises: PortalError: 404 app_not_found; 409 update_in_progress / 409 app_job_in_progress / 409 platform_not_ready / 409 app_running (the app must be stopped first). A failure after the job starts (bad manifest, disk full, sync failure) surfaces on the app's last_job, not here.
