Return one flat support document for name (design doc 3.2): versions, source, prechecks, logs.

Never contains a secret value or git token, even if an app printed one to its own journal -- the whole document is run through the same masking pass every registered secret value and git credential token goes through.

Raises: PortalError: 404 app_not_found.
