Update Agent

Changes name, description, scopes, expires_at, rate_limit_per_min or card (manager() only). expires_at and rate_limit_per_min accept an explicit null to clear. Narrowing scopes is a manager act; widening is fenced against the caller's own credential scope (403 insufficient_scope). A new expires_at in the future lifts a suspension for expiry. The slug is immutable.
