Resolve Employer

IMP-8: resolve a typed/picked employer into an affiliation.

{company_id: null} finds-or-creates the Company by normalised name (network_materialize_employer); an explicit company_id links to that already-chosen Company. Returns {company_id, created, affiliation_id} -- affiliation_id is the "{person_id}:{company_id}" pair identity, since contact_company_affiliations has no surrogate id column of its own.
