Update a skill (optimistic concurrency).

Updates any subset of the skill's fields. Requires the current version; a stale version returns 409. Every successful update bumps version. Editable by the owner, an ACL editor, or an org admin; changing visibility to public additionally requires skill:manage.

visibility'private' | 'shared' | 'public'

Promoting a skill to public requires skill:manage (enforced per-value in the handler regardless of the skill:write route floor; ADR-0061 §4, security review).
