End the session.

Clears the session cookie and redirects to the app.

Mode-conditional: when no dvt-owned session flow is configured (snowflake mode, or local dev with auth disabled) there is no session to end, so no handler is mounted and this path answers 303 to the SPA root instead (DVT-3463). Nothing is cleared and no sign-out occurs — in snowflake mode the ingress owns the identity.
