Health Check

Returns 200 if the tracker backend is alive and making progress. Docker healthcheck and external monitors hit this endpoint.

During backfill/catch-up after a restart, data timestamps will be old but the system IS working. We check that the backend process is alive AND that new rows are being inserted (last_id is advancing), rather than only checking timestamp freshness. This prevents Docker from restarting the container during normal catch-up.
