Add Quote Card

Founder: add a consultant quote-approval card (Wave 6A).

Creates a draft ci_da_consultant_rfqs row carrying the company, quoted price, description and contact. The card stays hidden from the customer until the founder clicks Surface. Founder-only; declared before /{project_id}.
