Github Webhook

Signature-validated GitHub push webhook.

A push to a connected repo+branch triggers a full snapshot refresh (clone + re-index) for every user connected to that repo. The refreshes run as background tasks and the handler returns 202 immediately — a full clone + embed can exceed GitHub's 10 s delivery timeout. Outcomes land in the log and in each connection's refresh_status / last_refresh_error, which Settings surfaces.
