Clear the caller's own avatar

Remove the caller's picture, so GET /users/{user_id}/avatar answers 404 and avatar_url disappears from their directory row. Gateway-native; the control plane's identity-provider default is unaffected. Self only. Idempotent: a caller with no picture answers 204 unchanged. The stored bytes are content-addressed and may be shared, so they are not reclaimed here.
