Tiering For User
P(TR | physician, case domain) for one physician.
case_domain is the whole point of the endpoint: the same physician is a different answer on a nephrology case and a cardiology one, and an admin deciding a tier needs to be able to ask both. Omitted, it defaults to the physician's own declared specialty.
