Detect Agent Trajectory

Agent 轨迹全链路检测：P1 分层管线（规则→裁判→搜索/重放）+ 幻觉传播追踪。

P1 重构：原逐步 3× _detector.detect(thought/observation/final_answer) 替换为单次 agent_pipeline.evaluate_trajectory 调用，单步 LLM 从 3 降至 ≤1。
