Cua Confirmations Stream

SSE 实时推送：新待确认/裁决完成时推送事件（载荷即最新 pending 列表）。

替代 3s 轮询的实时通道——事件触发前端立即刷新；连接断开时前端回退轮询。 注意：路由须注册在 /cua/confirmations/{cid} 之前，否则 "stream" 被 cid 捕获。
