Verification queue with duplicate-detection flags.

2026-06-09 — Hardened. The duplicate-id EXISTS subquery requires verification_requests.id_number_hash. If that column doesn't exist on this environment we fall back to a SELECT without the duplicate-detection flags so the queue still renders.
