Take an uploaded cover down (moderator)

Changed on

Moderators only (profiles.is_moderator, ACCESS_FORBIDDEN otherwise). Every post showing the uploaded image goes back to its default preset and the file is deleted; the post itself is untouched. A preset cover is left as it is (removed: false). 404 while EXCHANGE_BOARD_ENABLED is off.
