Create Feedback Addon Checkout

Start Stripe Checkout for the End-User Eval add-on (task 609).

Backend-driven: client sends only the SKU; backend resolves it to the env price id and never trusts a client price id. Entitlement is granted by the signed webhook on completion, not here.
