Update the coach's post-call CRM field mapping

Saves the field mapping. You'll get a 404 with crm_not_connected when there's no CRM to map into, so connect one first.

Session-only. This endpoint isn't on the API-key allowlist. An organization API key gets a 403 here, whatever scopes it holds.
