Put a removed installation back

A3: same rule as DELETE.../{id} (any non-observer member of the chat, or the personal installation's own owner), narrowed by security fix item 6, restated by role: refused when the caller is the APP'S BUILDER (app.built_by_id) and the removal was made by someone else (the builder may restore its own removal; anyone else who could have removed it may also put it back, but the builder alone may never quietly reinstate what another member deliberately took out). Refuses a blocked or deleted app outright. 409 with the existing one when another active installation of this app already exists in the same chat by the time this runs (pre-checked, and rescued again on a race past that check). A no-op (200, unchanged) when the installation was never removed; otherwise a restored installation moves to the app's latest version unless pinned.
