Add Test Correction Internal

The agent captures a live correction during a test-mode call (the tester telling it its CONFIGURED behavior is wrong). Persist-on-detect: one row per correction; the batch is reconciled and applied to the template at session end. Deliberately separate from session_notes so the agent's note-to-self ledger is untouched. See plans/pending/template-patch-agent-and-correction-sources.md.
