Get Decision Evidence

Return the provenance evidence rows for a single decision (0.13.0).

Each row mirrors the OSS DecisionEvidence shape (source, source_rank, evidence_file/line/commit, source_quote, confidence, verification). Legacy artifacts written before 0.13.0 carry no evidence array — those return an empty list rather than 404. 404 is reserved for an out-of-range index.
