Agent performance: per model and client, with the fix rate the agents reported

One row per (model, version, client) with checks in the period; calls that reported no model are the client's 'Model not reported' row, last. The rate is fixes reported by agent ÷ issues found and is not verified. Organisation totals; the rows sum to cards 3 and 4.
