Observations across this care-team member's children since their last visit per child.

Same single SECURITY INVOKER pass as the clinic feed - get_new_updates has no role-specific SQL, so child_profiles RLS scopes the caller (village_free care-team member) to exactly their claimed children. No new read-state plumbing: mark-read reuses /read/home/timeline-visited[-all].
