Report Resolution

A condition previously reported under body.reference is now observed healthy -- closes the loop on whatever open item matches (see supervision.record_resolution's own docstring for how it ties the clearance to the specific open item and refuses to fire twice). POST /report with urgent=True is what RAISES a defect/escalation; apps/rooster/entrypoint.sh:probe_brain calls this one only from the branch where its own probe actually answered "SEAT OK" -- never merely because nothing failed this boot.

resolved: false (with item: null) is the ordinary, expected answer on most calls: nothing was open under this reference, so nothing was posted -- silence, not an error.
