Set Caller Preference

Persist a PREFERENCE on the caller's profile — the HTTP equivalent of the MCP set_preference tool (the four-ops 'fill' verb that previously had no live endpoint). Wires straight to caller_context.set_preference; IDOR-safe (identity resolved server-side via _pref_caller). Returns {ok, caller, preferences}.
