Per-agent prompt-cache call counts and warm-miss counts.

A non-zero warm_misses for any agent means a 2nd+ call within this project showed cache_read_input_tokens == 0 despite the prompt being above the cacheable size threshold — the cache_control boundary for that agent is misplaced (or caching is silently off).
