List Smartadvocate Lookups

List the firm's mirrored SmartAdvocate picklist options for one kind.

kind is a registry kind (case_group, incident_state, office, ...); parent_sa_id scopes the parented kinds (case group for case_type, case type for case_sub_type) and stays 0 for the flat ones. Empty until the customer's sync_lookups toggle is on and a sync has run.
