Paginated CSV export (cursor-based)

Download mutations in pages of N rows using cursor-based pagination. Each response includes an X-Next-Cursor header with the CANLIB ID to pass as ?after= for the next page. Repeat until X-Has-More is 0. Example: page 1: ?gene=BRAF&page_size=50000, page 2: ?gene=BRAF&page_size=50000&after=CI-5184301
