Get the authenticated user's personal organization

Returns the authenticated user's personal organization — the auto-provisioned org-of-one that scopes their individual resources. Idempotent. If the user does not yet have a personal org (legacy account pre-dating migration 161, or a transient backfill miss), one is provisioned on the fly and just_provisioned is set to true.
