List Sent Email Groups

Accurate per-agent (campaign) roll-up for the Sent page headers.

Grouped over OutreachHistory (the same source the list reads), so each header's "N sent" is the true total for that agent — never a slice of a fetched window. Open-rate columns come from the left-joined EmailSend rows, matured on sent_at exactly like /outreach/stats. campaign_id NULL => the "Direct sends" group.
