Poll an escalated decision (grant receipt included)

Read the current state of an authorization request by id — used to poll an escalation until a human resolves it. Readable by the wallet's agent key or the management key. On approval the response carries the one-use grant the agent redeems by retrying with grant_id. Settles expired escalations to the policy's timeout action on read.
