Disconnect the caller's warehouse identity (revoke their grant).

Revokes the acting user's per-user OAuth grant on this connection (DVT-123) — a self-service "disconnect". A subsequent consent re-creates it. Revocation (not deletion) keeps the event auditable. Any member (revokes only the caller's own grant).
