Get Table Meta

Return table metadata.

With refresh=true and a live-query source, also introspects the remote schema and merges the result into the response in-memory — new remote columns are flagged status="new", stored columns missing remotely are flagged status="dropped". Nothing is written; persistence happens only when the caller subsequently PUTs /meta/columns.
