List all ICD history records for a claim.

Returns all versions ordered by version number descending (most recent first).

Each item carries `origin` — who produced that version: `ai_initial`,
`ai_revalidate`, `human` or `unknown` (rows written before provenance tagging).
Treat the set as open: new values may be added. `feedback` is the reason recorded
with that version, if one was given.

**Authentication:** API Key (X-API-Key header)
**Required Scope:** `claims:read`
