Mark Intraop Form Ready For Review

RN flips an IN_PROGRESS form to READY_FOR_SURGEON_REVIEW (PRD §4.4).

Validates required fields and OR-duration consistency, persists draft attribution, opens an escalations row tagged intraop:ready_for_review, and logs INTRAOP_FORM_READY_FOR_REVIEW.
