Annotation Queue

The ENV annotation queue (PRD §7.2). Gated on portal_version == 'env'.

allow_legacy: a client cached before the rename still sends 'v5'. It is accepted for one release rather than 400ing a physician mid-annotation over a string their page will stop sending the moment it reloads. Nothing is STORED from it — the response echoes ENV_PORTAL_VERSION.
