Annotate Environment

A board-certified physician submits the §7 annotation for one trajectory (run). Gated on portal_version == 'env' (PRD §12.9).

Legacy 'v5' is accepted for one release — see the queue above. It is never written: an env annotation lands in env_runs, which has no portal_version column, so there is no way for the legacy string to be mistaken later for a longitudinal submission.
