Record sandbox provisioning approval gate

Records an owner/admin approval artifact consuming a ready sandbox provision preflight plus owner/operator approval, no-op switch, activation gate, provider/runtime/image, budget, rollback, receipt, and logging refs. Requires write:true and idempotency_key. It does not provision or execute anything.
