Commit To Prepare

Create or update a user's preparation commitment for a Pulse event.

Idempotent on (user_id, activity_id) — calling twice with different values updates the existing row. Only the fields actually passed in the body are written; nulls preserve existing values.

Snapshots wearable_linked at commit time so the generated plan in Milestone 3 knows whether to reference movement data.
