Create Consent Record

Create a consent record for a claim.

A contact channel (sms, voice, email) is the claimant's contact consent (conversations/consent.py): a person records it and says how it was obtained (obtained_via; 422 without), granted: false records a withdrawal; refused (409) for texts or calls after the claimant texted STOP. It then counts for outreach.
