Detach an alert channel from a monitor

Removes one alert channel from this monitor's routing set and voids that pair's pending alert jobs, returning the updated monitor. The dispatcher's claim joins alert_jobs to alert_channels and never re-checks monitor_alerts, so an un-voided pending job would still be delivered through a channel the customer just switched off for this monitor. Detaching a channel that is not attached is a no-op success; the 404 is reserved for a missing or foreign monitor.
