Assist Writing Fix

Propose the SMALLEST patches that resolve the writing findings, using the editor model (Sonnet-class, not the lite tier). Returns find/replace pairs keyed to findings — it never rewrites the document and never applies anything; the caller (the user's Claude) applies what the user accepts and re-runs review_writing. Numbers are never invented: an unsupported claim is qualified, marked [SOURCE NEEDED] or removed. Costs WRITING_FIX_COST credits per call; a detector-only pass (no findings) costs nothing.
