Open Case For Identity

Open a review case anchored to this identity.

Phase 4 of identity-centric refactor — cases are about people, so we expose a direct POST /identities/{id}/cases path. If session_id is supplied, the case is scoped to that session as well (useful when the case was triggered by a specific attempt); otherwise the case is purely identity-level.
