Coach Active Session

Start the session (AC6)

Transitions a scheduled group session to in_progress. Enforces the T-15 readiness gate (per AC6 + MED-791 AC1). Initializes current_section_index = 0 so AC5 timers start accumulating.

Idempotent: calling on an in_progress session returns the current state with 200 rather than 409.
